Nearly all disorders that initially or predominantly present with oral lesions may also manifest outside the oral cavity as prototypically seen in lichen planus, Behçet disease, and pemphigus vulgaris. In some disorders, extra-oral presentations give valuable clues to the diagnosis such as involvement of nails and scalp, sites that are easily acessible to every physician. In many others, e.g. in Crohn’s disease, Cowden syndrome, mucous membrane pemphigoid, syphilis, gingival hyperplasia, malignant melanoma, and Kaposi sarcoma, the presented oral pathology may be the starting point for a thorough examination of the patient. In some of our patients, the oral lesion may fnally prove only to be the tip of an iceberg with relevant systemic disease or major pathologies at other sites.
